After a stay in hospital, the Urgent Response Service aims to help you maintain or regain your ability and confidence to live at home.
The team will provide support to help you develop new ways to manage and to do things differently. It is about doing things with you, rather than doing things for you, so that you can remain in your own home as independently as possible. Support may only be for a few days, or perhaps longer, but can only be received for a maximum of 6 weeks.
The service operates between 7am and 10pm, 365 days a year.
How do I receive support?
A referral can be made via the hospital discharge team or adult contact team by you, a member of your family and/or representative as well as health and adult care service professionals.
To receive this service you must:
- Have an adult service community care assessment
- Be 18 years old and over
- Be living within Southampton City Council’s boundaries
