Supported housing

To find out if supported housing is right for you, you should first talk to friends, family, your key workers, advocates and your care manager.

Who can use this service:

If you have a learning disability and need help and/or care to live on your own, you may be eligible to apply for Supported Housing. There is a criteria which the customer needs to meet. This criteria shall be fully explained by the assessment team. Customers need to be assessed before they they can use this service. Customers can apply by contacting the Health & Community Contact Centre, or your care manager. This service is only available for people living within the city boundary.

Charges:

There is a charge associated with this service. There is no charge for the assessment but some services offered do carry a charge. This will be explained to you at the assessment. The customer will be invoiced for these charges.

Ways To Pay:

Customers can pay for the charges by cheque or postal order

Details of service:

If you decide that want to move into supported housing, you will need to complete an assessment with your care manager.

The Assessment:

To make an appointment for an assessment, please contact Health & Community Care Contact Centre. During the assessment, we will try and find out the levels of help and / or care you will need, for example:

  • Help with money management

  • Help cooking

  • Help looking after your home

  • Help with your personal care

We will also talk to your friends, family and current care staff to assess the correct level of support you will need to live alone. We will also find out where you want to live and what type of house you want to live in.

Our assessment team will help you look or the housing that best suits your needs, we will find you the right support workers, for the amount of time you need them.

It can take some time for us to find you a suitable accommodation, your care manager will keep you informed as to all developments.

This could include moving to sheltered accommodation or being part of the Community Alarm System (CAS) also known as Careline.
