The Alerter process
This gives providers guidance and timescales for action.
Safeguarding Alert Form 1
This form contains guidance for staff and is set out to enable details of any safeguarding incidents or concerns to be formally recorded. The intention is that the Alert Form 1 is completed by the staff member and passed immediately to the provider manager on duty for them to action. Completion and retention of this form evidences that Safeguarding issues have been alerted within the provider service.
Safeguarding Alert Form 2
This form contains guidance for provider management staff and should be completed by them and forwarded to the Health and Adult Social Care Contact Centre without delay.
We recommend that you ensure that all staff/employees/volunteers are made familiar with the process and that the Alerter process itself is clearly displayed as a reference chart for your staff. Additionally, all staff have access to the Alert Forms relevant to their role.
Safeguarding Adults referrals can be made to the Adult Contact Centre at any time but we recommend that the above documents are utilised wherever possible.
