Root cause analysis is a systematic approach to investigating serious events within any organisation, designed to establish the key events which led to the serious incident occurring.
It may be that a range of factors have led to a serious mistake or error occurring. The purpose of the root cause analysis is to examine these factors in a balanced and thorough manner enabling the management team to learn important lessons rather than to apportion blame to a single individual.
Completion of the root cause analysis is frequently requested from providers alerted to the Safeguarding Team.
