When a serious incident occurs to an adult, known to have been at risk of harm, a serious case review may be agreed as an appropriate additional part of the investigation.
An inter authority procedure has been agreed for serious case reviews within Southampton, Hampshire, Portsmouth and the Isle of Wight.
