How care and health services work

Who do we help?

We provide help and support to adults with physical disabilities, learning disabilities, severe mental health problems and chronic long term illness. We can also help if you are are blind or partially sighted, hard of hearing or deaf, experiencing problems with substance misuse or living with HIV. We also offer support to people who are caring for a relative or friend.

Who is eligible for our services?

To ensure our resources are fairly allocated, we have to limit our support to those with the greatest need. You must be over the age of 18 and living in Southampton. Our services may also be available to you if you are a patient in a Southampton hospital and no other authority has responsibility for your care. Making an assessment

If you are eligible for our support we will carry out an assessment to find out what help you need. This can take place in your home, at a hospital or one at of our offices and you are welcome to bring a friend or relative. We use this information to identify what support you need.

During the assessment we look carefully at your personal circumstances, the extent of any impairment, what things you can do and those that are causing difficulty. We will talk to you about any help you directly receive, and whether anyone else, such as friends or family members could help you.

If a lot of help is needed, we may arrange a joint or further assessment with someone from another organisation, such as a housing officer or occupational therapist. All assessment records are stored on a computer that only authorised staff can access. All information is treated as confidential unless we are given express permission to discuss it with others.

What happens next?

Once we have agreed the help we can offer, we will write a care plan. The plan will give full details of the services you will receive and when. Your care manager will coordinate the delivery of the services, even when they are being provided by another organisation. They will carry out regular visits to ensure that the care plan continues to meet your needs.
