The following forms should be completed by the manager or deputy of a care home (the managing authority) and sent to the DOLS Administrator of the supervisory body (Southampton City Council).
• Form 1 is completed by the care home to grant themselves an urgent authorisation to deprive a resident of their liberty for 7 days. At the same time, Form 4 must also be completed and sent to the supervisory body.
• Form 2 is to be completed to extend the urgent 7 day authorisation. This will only be accepted by the supervisory body in exceptional cases
• Form 4 is to be completed to request a standard authorisation from the supervisory body
• Form 14 is for a care home to notify the supervisory body that a standard authorisation should be suspended because the person no longer meets the eligibility requirement
• Form 15 is for a care home to notify the supervisory body that the eligibility requirement is again met and the suspension can be lifted
• Form 19 is for a care home to request a review of the standard authorisation by the supervisory body
• Letter 1 is for a person to send to the care home if they are concerned about a possible unauthorised deprivation of liberty (i.e. the person has not yet been assessed by the Supervisory Body and their liberty is restricted or deprived so that they cannot make the choices they would like about their way of living)
• Letter 2 is for a person to send to the supervisory body if they are concerned about a possible unauthorised deprivation of liberty
• Letter 3 is for a person subject to a standard authorisation to send to their supervisory body to request a review of their authorisation
• Letter 4 is for the representative of a person subject to a standard authorisation to send to the supervisory body to request a review of the authorisation
