City Care First Support

Southampton City Council operates a rehabilitation and reablement service. City Care First Support will aim to help you maintain or regain your ability and confidence to live at home.

The team will provide support to enable you to develop new ways to manage and to do things differently. It is about doing things “with you”, rather than doing things “for” you, so that you can remain in your own home as independently as possible. This may be for as little as a few days or perhaps longer but only up to a maximum of 6 weeks.

The service operates between 7 a.m. and 10 p.m. 365 days a year.

How do I receive First Support?

A referral can be made via the hospital discharge team or adult contact team by you, a member of your family and/or representative as well as health and adult care service professionals.

To receive the First Support service you must:

  • have an adult service community care assessment
  • be 18 years old and over
  • be living within Southampton City Council’s boundaries
