The rehabilitation teams are made up of nurses, care managers, physiotherapists, occupations therapists and other care professionals. They provide short term help to make sure that you remain as independent as possible if you have a short term health need.
Locality teams
The locality teams ensure that longer term follow up support is available if you need it. They work with GPs, community nurses and other health professionals.
Hospital discharge team
The hospital discharge team work with you before you are released from hospital. They can help you return to independent living or find suitable long term care if needed.

The elderly mental health teams provide specialist support for people suffering from mental health problems such as dementia.
