Apply for a Smartcities card

Please complete an application form and return it in person to any local library, local housing office or Gateway in the Civic Centre.

Please note that all applicants aged 14 years and under will require a counter signature from a parent or guardian.
You can obtain an application form by visiting Gateway, a Southampton library or local housing office or by using one of the links to the right to either download a printable application form, or to receive one by post.

Along with your form, you must provide proof of your address on one current document in your name, not more than three months old from one of the following:

• Utility bill (dated within the last 3 months)
• Bank statement (dated within the last 3 months)
• Council Tax bill (current)
• Official correspondence

Please also provide one proof of personal identification from the list below:

• Driving licence
• Passport
• Birth Certificate

Applicants under pensionable age applying for a concessionary bus pass will need to provide one proof of qualification as stated in section 4 of the Smartcities application form. A copy of this document must be attached to the application form.

Applicants for the Get Active initiative must provide proof of being in receipt of one of the benefits listed in section 5.

For PASS Proof of Age applications please attach a copy of your personal identification document which has been signed and dated by your verifier. A proof of address is also required and a verified photograph.

A colour passport photograph taken on a light background will be required. Digital or black and white photographs cannot be accepted. However, Gateway is now able to take your photograph for use on your Smartcities card. This is not a guaranteed facility as there may be complications which prevent us from offering this service.
