Individual Application

Please complete three sections of the form as follows:

  1. Section 1 – Information about you
    Section 2 – Questions to help us assess your application

  2. Please select the questions that apply to you:

    Section 3 – Questions for applicants with walking difficulties
    OR
    Section 4 – Questions for applicants with a disability in both arms
    OR
    Section 5 – Questions for applicants under the age of three

  3. Section 6 – Declarations and signatures

Please ensure that you fill in all fields marked with 'required' as these are mandatory in order to progress and complete the form.

The final page of the application will provides instructions on where to send the remaining evidence documents.

Organisational Application

This application is intended for organisations involved in the care of disabled people who are seeking a Blue Badge for vehicle or vehicles (e.g. minibus or specially adapted commercial vehicle) which is/are to be used to carry disabled people who would themselves qualify for an individual Blue Badge. Please see the guidance notes, link below, for a list of the eligibility criteria prescribed in the regulations that govern the scheme.

Your Successful Application

If your application is successful we will contact you (or your nominated representative for organisations) to arrange a suitable date for collection of the Badge(s).

On the day of collection you must go to Gateway, One Guildhall Square, Southampton, SO14 7FP, and bring:

  • 1 form of photo ID i.e. passport, driving licence, bus pass.

  • In the case of re-applications, your existing Badge.

  • A fee of £10 per badge. This can be paid by cheque, postal order or cash. Cheques and postal orders should be made payable to Southampton City Council.

  • Organisation only - please also bring with you proof of your representation of the organisation

You will not be issued with your new Badge(s) if these are not presented at the time of collection from Gateway and you will then need to contact the Blue Badge team on 023 8083 3748 to re-arrange a suitable collection date and time.
